Now is the time to join or make changes to your Sick Leave Bank enrollment.

Requests to enroll, change or withdraw must be in the BTF office no later than Friday, June 20, 2008 by close of the business day (5:00 pm).

To enroll: Complete the enrollment and authorization cards (pink AND manila) See your building delegate or contact SBF for enrollment cards.

To change: Complete the enrollment and authorization cards (pink AND manila)
See your building delegate or contact SBF for enrollment cards.

To withdraw: You must withdraw by written request. This request must be forwarded by certified mail only. Faxed copies are not acceptable.

To confirm that your Sick Leave Bank cards were received and processed, you may contact the SBF office at 881-5462 after August 1, 2008.
